Download the forms needed for MembershipPLUS related risk management planning.

Note: Some forms are in PDF format for printing and easy use offline (eg. while at an event). Other forms/documents are in MS WORD format and may be edited, eg. to add the club name, before being printed and used at a meeting or event.

For more information about how they are used, refer to the AOMC MembershipPLUS Handbook.

  • Note: Most of these forms are PDF format when it is intended for an AOMC managed process.

    For those forms that need to be returned for processing (eg the Application Form) please note the forms are not “fillable” forms, so either:

    1. edit the PDF using an editing tool like Acrobat Reader, save the form and then email it, or

    2. handwrite on the PDF and scan it or photograph it and then email it, or

    3. handwrite on the PDF and post it

    Some of these download templates are Word .docx format that enable clubs to edit the document and create club specific risk management plans

Use this form when applying for MembershipPLUS affiliation.

2. Event Sanction Request Form

Use this form to apply for an event to be sanctioned.

3. Emergency Plan Template

Use this form to record emergency plans for meetings and events.

4. Event Checklist - Car Display Sample

Use this checklist as a template when preparing a risk management plan for a public vehicle display day. It can be modified to suit your event.

5. COVIDSAFE Plan Template

Use this Government recommended COVIDSafe planning template when required or use it for preparing other pandemic management plans

6. Incident Report and Analysis Protocol

This document explains the purpose and use of the following Incident Report and Risk Analysis forms.

7. Incident Report Form

Use this form at an event to record details of a safety incident.

8. Risk Analysis Form

Use this form to record the risk analysis arising from a reported incident and the corrective action plan, if needed.

9. Event Attendance Form Template

Use this form as a template to capture attendance of members at meetings or events, including the identification of those who were volunteers at the event.
